The Community Health Access Project (CHAP) provides wraparound services to members of the Crawford and Morrow County communities, helping bridge gaps in healthcare access and connecting individuals and families to essential resources.
Through education, advocacy, and direct support, CHAP promotes overall well-being, stability, and empowerment for those we serve.
Community Health Worker
Our team of compassionate and trained Community Health Workers acts as trusted guides and advocates — helping participants navigate local resources and connect to vital support systems.
They can assist with:
- Access to essential needs such as food, shelter, clothing, and transportation
- Training and educational materials tailored to individual and family needs
- Referrals to local health and social service agencies
- Advocacy and outreach to ensure no one falls through the cracks
- Enrollment support for available community programs
- Social-emotional support and encouragement
- Navigation and connection between participants, families, and services
